Please Personal Data Protection Law (“KVKKIn accordance with ”)’na, our company Evly Pharma Cosmetics San. and Tic. A.S. (“Company”)To fulfill your request, please send us the following application form by filling it in a clear and complete way.

We will reply to your application as soon as possible according to the nature of the request and within thirty days at the latest. If the information and documents you provide to us are incomplete or incomprehensible, we will contact you to clarify your application.

Our Company reserves the right to request additional paperwork and information for identification and authorization determination, in order to eliminate legal risks that may arise from unlawful and unjustified data sharing and in particular to ensure the security of your personal data. In the event that the information regarding your requests submitted under the form is not accurate and up to date or an unauthorized application is made, our Company does not accept liability for such incorrect information or unauthorized requests. All liability arising from unlawful or unlawful, misleading or erroneous applications belongs to you. Within the scope of the Communique on the Procedures and Principles of the Application to the Data Controller, if our response to the application you have made exceeds 10 pages, 1 Turkish Lira processing fee will be applied for each page after the 10th page.
